Does Your Dental Plan Work? |
| |
Our Dental
Care Plan is a dental referral organization that offers you memberships
in its discounted dental plan. As a member, you can save 25% to 65%
on all restorative and cosmetic work (fillings, crown, braces, etc.)
and up to 80% on preventative work (teeth cleaning, x-rays, etc.)
performed by a general dentist. Specialist's fees are discounted 25%. |

How
do I use the plan? |
| |
Within
10-14 days of your enrollment, you will receive a membership information
guide, membership identification cards, and a Dental, Pharmacy, Chiropractic
Care and Vision Care Directory. Just present your membership identification
card to any of our providers and you will eligible to get the discounted
fees. You pay the provider at the time of the service. |

Who
is included in the family plan? |
| |
The family
membership plan includes all residents in a household, including children,
parents, relatives, significant others, and all permanent members
of the household. |

Are
Pre-Existing Conditions Covered? |
| |
Since
our dental plan is not insurance or a health care organization, all
ongoing dental problems are accepted except orthodontic treatment
in progress. |

Are The Dental, Pharmacy, Chiropractic Care and Vision Care Providers? |
| |
Our
dental program has assembled one of the largest networks of dental
providers (over 30,000 dentists) in the nation, and we are continually
adding to that base. The dentist are well-established with successful
dental practices. Our Prescription Card may be used at over 50,000
pharmacies nationwide as well as through mail order. The Vision benefits
are available nationwide at more than 12,000 providers. We have accumulated
over 7,500 leading chiropractors to provide services to our members.

there any deductibles or claim forms to fill out? |

There
are no deductibles, no claim forms to fill out, and no limit on visits
to your dentist. |

Is
there a waiting period before I can use any of the services? |
| |
You may
start saving money with the Dental Plan immediately. The Vision, Prescription
or Chiropractic Plans can be utilized the day you receive your membership
card. |

How
Do I Pay My Membership Fee? |
| |
All monthly
or quarterly payments can be made by an electronic bank draft system
or by credit card. Invoicing is done only for annual memberships paid
one year in advance. Individual memberships are only $14.95 per month,
and family memberships are only $19.95 per month. |
